You may reproduce this spreadsheet (use landscape mode in printer setup). It works well to have it 11" x 17" and on heavy paper, supported by a clipboard. This can be reduced to 8.5" x 14" by setting the copier at 75%. The larger size is better for recording. You may have your hospital ED demographics printed on the top. On back side it may be helpful to have body diagrams (e.g. burn/trauma).

A computerized version - has not been developed, but is not far away - given some dedicated users. Please indicate your interest level.
